A stroke is one of the leading causes of disability in the United States. Following a stroke, 40% of stroke patients are left with moderate functional impairment and 15% to 30% have a severe disability as a result of a stroke. A neurogenic cognitive-communicative disorder is one result of a stroke. Neuro- meaning related to nerves or the nervous system and -genic meaning resulting from or caused by. Aphasia is one type of a neurogenic cognitive-communicative disorder which presents with impaired comprehension and production of speech and language, usually caused by damage in the language-dominant, left hemisphere of the brain. Aphasia is any disorder of language that causes the patient to have the inability to communicate, whether it is through writing, speaking, or sign language.<br />

<span style="font-size: large;">Symptoms/Language Characteristics</span></h3>
<br />
<a name='more'></a>Mixed transcortical aphasia is characterized by severe speaking and comprehension impairment, but with preserved repetition. People who suffer mixed transcortical aphasia struggle greatly to produce propositional language or to understand what is being said to them, yet they can repeat long, complex utterances or finish a song once they hear the first part. Persons with mixed transcortical aphasia are often nonfluent, and in most cases do not speak unless they are spoken to, do not comprehend spoken language, cannot name objects, and cannot read or write. However, they often have the ability to repeat what is said to them. In fact, persons with mixed transcortical aphasia often repeat in a parrot-like fashion. Some patients with this disorder can experience many different types neurological symptoms including, bilateral paralysis, lack of voluntary speech, and difficulty with producing spontaneous speech.<br />

A conversation between a clinician and person with transcortical mixed aphasia would have similar characteristics to the conversation below:<br />

In this rare type of aphasia, Broca's area, Wernicke's area, and the arcuate fasciculus are intact but the watershed region around them is damaged. This damage isolates these areas from the rest of the brain. The most frequent etiology of mixed transcortical aphasia is stenosis (narrowing) of the internal carotid artery. Mixed transcortical aphasia can also occur after cerebral hypoxia, cerebral swelling, and any stroke that affects the cerebral artery. Often lesions that cause mixed transcortical aphasia affect both the anterior and posterior perisylvian border zones. Some times the type of aphasia can be determined just by knowing the lesion location. In order for a patient to be diagnosed with mixed transcortical aphasia all other forms of transcortical must be ruled out. Using WAB or the BDAE can rule out global aphasia if the ability to repeat is present. If verbal fluency is depressed transcortical aphasia gets ruled out and if auditory processing and comprehension is weak then it cannot be transcortical motor aphasia.<br />

<span style="font-size: large;">Treatment and Therapy</span></h3>
<br />
After a stroke, many patients feel the devastating impacts of the loss of language. Studies have looked into ways to enhance verbal communication with therapy, and one of the treatment approaches that proved to be successful is "<i>Drawing Therapy</i>". Drawing offers an alternative route to access semantic information. Because of this, it provides adults who have lost language with a means to access and express their ideas, emotions, and feelings. Drawing has also been shown to activate right hemisphere regions. This makes drawing a non-linguistic intervention that can access semantic knowledge in the right hemisphere. The study conducted on drawing therapy found that it increased naming abilities in patients with acute and chronic aphasia. It also produced fewer error attempts during naming tasks. The study also found that the act of drawing itself, not the quality, was critical for the activation of the semantic-lexical network required for naming tasks. Other studies have also reported that family members have seen the effects of therapy at home. The gains made from drawing therapy were not ones that could have been made from spontaneous recovery. Drawing therapy was especially found to be useful in individuals with global and anomic aphasia. Both of these individuals were found to have produced more verbalizations post-therapy. This reinforces the idea that drawing provides a mean of recruiting areas or networks that were not otherwise sufficient for producing speech. Across the majority of patients, the quality of drawing improved as well as written output and sentence structure. Drawing therapy has proved to be effective even after a few periods of therapy.<br />

Although this therapy is aimed at patients with aphasia, it can be implemented for any patient with expressive deficits.<br />
<br />
Drawing therapy can be implemented in the following hierarchy:<br />
<ol>
<li> Clinician gives patient a prompt and asks them to draw a response</li>
<li> Clinician asks for clarification of drawing if it is unclear (add more detail, enlarge one aspect of the drawing)</li>
<li> If possible, patient verbalizes about their drawing and assigns language</li>
</ol>
<br />
Drawing therapy can also be made harder through the type of prompt given and the task difficulty. Prompts and questions that are opened ended and with broad semantic categories are going to be harder to conceptualize and draw. It is a harder task when the client is asked to draw abstract concepts or sequenced actions/events.<br />

Overall, drawing therapy offers a means of accessing language to patients with aphasia who have difficulty expressing themselves with different modalities.<br />

<span style="font-size: large;">Assessment</span></h3>
<br />
For most patients, formal and informal language assessments are initially administered during his/her acute hospital stay by a licensed speech-language pathologist. However, a standardized assessment may provide further information regarding an aphasia classification. While there are different classifications of aphasia (i.e., Broca’s, Wernicke’s, Conduction, Anomia), they each have hallmark deficits. Research has shown, a patient presenting with mixed transcortical aphasia will have impairments in all communicative areas, with the exception of the preserved ability to repeat a person’s words or phrases. Patients with mixed transcortical aphasia demonstrate similar deficits as those seen in patients with global aphasia. Therefore, assessment of repetition is most critical in order to differentially diagnose.Specifically, language based standardized assessments such as the Western Aphasia Battery (WAB), and the Folstein Mini Mental State Exam include a repetition subtest amongst all other language-related areas.<br />

Other possible assessments that can provide further differentiation include: Magnetic Resonance Imaging (MRI) Magnetic Resonance Angiography (MRA) CT scan Informal observation<br />
<br />
It is important to remain diligent in providing therapy regardless of aphasia classification.<br />
<br />
<h3>
<span style="font-size: large;">Prognosis</span></h3>
<br />
If brain damage is minimal then a patient may recover language skills overtime without treatment, however if the damage is severe it may be necessary to receive speech and language therapy. Recovery from this type of brain injury is a slow process and very few patients regain the same level of language and communication skills that they have before the injury. Patients with aphasia usually undergo speech therapy where they relearn and practice supplementary communication methods. Speech therapy is not a cure for the aphasia, but instead helps patients use skills that remain intact. When considering the prognosis for individuals with aphasia it is necessary to consider internal factors, patient specific factors, and external factors as these factors are considered most critical to post-stroke recovery. Internal factors are factors related to the stroke such as aphasia severity, lesion site and lesion size . Individuals with milder forms of aphasia, lesions that insignificantly impact language function and smaller lesions tend to have a higher degree of aphasia recovery. Lesions in the superior temporal gyrus (STG) produce a more persistent global aphasia, which is associated with poor aphasia recovery. Patient specific factors relate to the patient's age of onset, education level and motivation for recovery. Younger patients have been reported to demonstrate a higher recovery rate than older patients. Those with more years of education are less vulnerable to language disruption by stroke. External factors include environmental factors such as type and amount of language treatment provided. Stroke patients who are generally aware of their handicap and receive good support show more motivation and are more likely to have a better outcome.<br />

"We report two patients with mixed transcortical aphasia following left frontal lobe infarctions. Although there was no evidence of anatomic isolation of the speech area on computed tomograms or magnetic resonance imaging scans, single-photon emission computed tomogra- phy in one case demonstrated diminished blood flow over the left parietal convexity suggestive of "functional isolation" of the posterior perisylvian language zone. (Stroke 1990^1:953-956)<br />

Mixed transcortical aphasia is characterized by reduced or absent spontaneous speech, severely impaired language comprehen- sion, and preserved repetition. Although the localization of lesions producing mixed transcortical aphasia is variable, the syndrome is seen most often with diffuse or multifocal pathologic changes that involve both anterior and posterior left hemisphere cortical association areas but spare the perisylvian language core. According to Goldstein, the critically placed lesions produce an "isolation of the speech area" posteriorly from the "ideational field" of the parietal association cortex and anteriorly from frontal areas important for the production of volitional speech. Since Broca's area, Wemicke's area, and the arcuate fasciculus are usually spared, repetition remains intact.<br />
<br />
Acute mixed transcortical aphasia is uncommon. Recently, Bogousslavsky et al described four cases of acute mixed transcortical aphasia in the setting of internal carotid artery occlusion with computed tomographic (CT) evidence of simultaneous anterior precentral-central sulcus artery territory embolic infarction and posterior watershed infarction related to hemodynamic insufficiency. These cases support Goldstein's contention by demonstrating both ante- rior and posterior anatomic isolation of the speech area.<br />

We report two cases of mixed transcortical aphasia in which CT and magnetic resonance imaging (MRI) showed only frontal left hemisphere infarcts.<br />

The motivation to seek social contact may arise from either positive or negative emotional states, as social interaction can be rewarding and social isolation can be aversive. While ventral tegmental area (VTA) dopamine (DA) neurons may mediate social reward, a cellular substrate for the negative affective state of loneliness has remained elusive. Here, we identify a functional role for DA neurons in the dorsal raphe nucleus (DRN), in which we observe synaptic changes following acute social isolation. DRN DA neurons show increased activity upon social contact following isolation, revealed by in vivo calcium imaging. Optogenetic activation of DRN DA neurons increases social preference but causes place avoidance. Furthermore, these neurons are necessary for promoting rebound sociability following an acute period of isolation. Finally, the degree to which these neurons modulate behavior is predicted by social rank, together supporting a role for DRN dopamine neurons in mediating a loneliness-like state.<br />

Packing up to move is nobody's idea of fun. In fact, it's one of the top ten stress causing items on the list of major stress factors that heavily impact your well being. It's near the top like death of a spouse or loved one. Lord knows, I was already up to my eyeballs with that stress factor.<br />
<br />
Moving after a stroke is no picnic.What to take. What to sell. What to donate. What to trash. All these decisions were up to little, old me. ALONE. Before, it was a couples or family thing. To top it off, I was seriously down sizing. I am going from an over 2,000 sq ft house to one less than 200. One-tenth and I had stuff galore to fill my 2,000+ sq ft when you count the attic, and out buildings. For weeks before the move, I never felt so ill prepared for a move in my entire life!<br />

I was hit full force with what I could do and what I could not do. There was a whole lot I just physically could not do. It wasn't that I wasn't trying. The spirit was willing, but my body couldn't. That's a bitter pill for my never-say-die attitude. To fill one box took about forty trips from where whatever it was was to the box and back again. The fact that my left side was weaker, yes even after almost four years, than my right before my stroke was a major tell. I could only carry a few things at a time to be placed in the box. That's if I didn't want to leave a trail of items between one place and another. That meant small loads, one or two items at best.<br />

<a name='more'></a>Compound this issue with my fatigue and my Botox wearing off, it took a good amount of effort to fill one box. Then, there was the question of what to do with the box when full. I found that if the box wasn't to heavy or too big, I could pick it up in a football carry and take it to the staging area. I had rooms set aside for "take," "donate," "storage," and trash." I had to rest between boxes and still it was a maddening cycle. If the boxes were to heavy or big, they had to stay where they were. Talk about an obstacle course!<br />

I was fit to pull my hair out several times over the month. Yes, it took a month at the snail's pace I was doing it at. I wasn't having fun. You know me, I always try to make everything enjoyable to keep going. I was totally opposite of how I am usually. I was irritated, short tempered, and didn't see the redeeming quality in anything. In fact, I seriously considered lighting a torch and setting the whole house aflame several times. I couldn't stand to be around myself and there's no way escaping yourself. I could see myself. Hating myself. But unable to stop myself from doing it. That's horrible for me.<br />

One morning it dawned on me. Quite literally because the spastic muscles in my arm don't let me sleep long. The pain isn't as great with the dry needling, but it will still wake me up to reposition my arm. What am I doing and why am I hurting myself? I needed to accept what is and give myself permission to fail and not do. So everything is not packed right, or folded right. It wasn't as if I was moving across country or another country for that matter. If it got broken, I'd glue it back together or get a new one. If it got wrinkled, I'd straighten it after I got there. If I couldn't lift or carry something, I'd put a colored code sticky note on it for others to deal with. I did hire some movers after all. For the stuff that was going to storage, there were some beefy, college students and family. I didn't NEED TO DO IT ALL.<br />

That's the major malfunction with a person like me. It's a control, stubbornness, and pride thing. The can-do attitude just doesn't know when to quit. I've never accepted failure as an option. I had to give myself permission to fail and not do. We, as human beings, are our own worse critics. It's alright to not do some things that are physically impossible (within limits). This revelation goes against my "nothing is impossible" sign off. Some things are. Like trying to lift a 24" square box, weighing thirty pounds, balancing it, carrying it from one room to the next, and setting it down without dropping it with one working arm. It's a whole lot ungainly than a fifty-pound bag of rabbit pellets or a bale of hay. Yes, I can do that and do it quite regularly. The contents won't break if I drop it. It's also easier to hold onto.<br />
<br />
After this revelation, I started feeling better about myself. It's crazy that I have to gibe myself permission, but it worked. I was calmer, more pleasant to be around, and at peace once again. I found my equilibrium once again. Moving away and setting up a new household is stressful by nature. The move wasn't without problems, but I could take it in stride once I'd given myself permission to fail. Lesson learned.<br />

On a sad note, I have to say good-bye to the Stroke Survivor Tattler. After almost three years of a republishing agreement, they are closing their website. They were based out of Canada and was my first international republishing contact I had. It had grown by leaps and bounds since I found them. From the dozen or so bloggers and three expert articles on various aspects of stroke/brain injury survivors to over forty bloggers and ten experts. Hits wise it jumped over 2K a month. John Anderson and the rest of them will be missed. I'm proud to be part of such endeavor and they will forever be friends who were there when I needed them most.<br />

Upon hearing that her elderly grandfather had just passed away, Katie went straight to her grandparent's house to visit her 95-year-old grandmother and comfort her.<br />

<b>Uber</b> didn't start out with grand ambitions to cut congestion and pollution. But as the company took off, co-founder <i>Travis Kalanick</i> wondered if there was a way to get people using <b>Uber</b> along the same routes to share rides, reducing costs and carbon footprint along the way. The result: <b>uberPOOL</b>, the company's carpooling service, which in its first eight months took 7.9 million miles off the roads and 1,400 metric tons of carbon dioxide out of the air in Los Angeles. Now, <i>Kalanick</i> says carpooling could work for commuters in the suburbs, too. <a name='more'></a>In early January 2011, I set my goal for to do something useful at least for +/- 5 years on the web. I got <a href="https://en.wikipedia.org/wiki/Blogger_(service)">blogspot</a> from <a href="http://google.com/">Google</a> and my site is <a href="http://stroke-survivors.blogspot.com/">stroke-survivors.blogspot.com</a> (and shorten to <a href="http://stroke-survivors.org/">stroke-survivors.org</a>). <br />
<br />
At first it is awful i.e. <b>not useful at all,</b> but in early January the thought the title is "<a href="http://www.stroke-survivors.org/search/label/%CF%B4%20Index"><i><b>Saturday News</b></i></a>" and might be <b>useful</b> indeed! January 28, 2012, I published <b><u>Saturday News</u></b> <a href="http://www.stroke-survivors.org/search/label/%E2%96%B7%20Jan-28-2012">Broca's Aphasia - Sarah Scott</a> plus <a href="http://www.stroke-survivors.org/search/label/%E2%96%B6%20Comics">Saturday Comics</a> and three stroke articles. Today, <a href="http://stroke-survivors.org/">SSTattler</a> published every week, usually about 30 articles+/-, except during summer holidays (July & August).<br />

On February 1, 2015, I read <a href="http://www.myhappystroke.com/2015/02/ground-hog-day.html" target="_blank">Ground Hog Day</a> on <a href="http://www.myhappystroke.com/" target="_blank">My Happy Stroke</a> by <a href="https://www.blogger.com/profile/01728427612743156573" target="_blank">Grace Carpenter</a>. In her article, she said "<i>On this anniversary, I'm going to make it official: I'm taking a break from blogging...</i>". Her blogs from February 2, 2010 to February 1, 2015 - exactly 5 years.<br />

Hmm, her statement is very nice because I was a bit tired as well for <a href="http://stroke-survivors.org/">SSTattler</a>. So, I set my goal to quit the next year, 2016, enough is enough and my goal to the end <a href="http://stroke-survivors.org/">SSTattler</a> June 25th / 2015 - total is 5 years and 6 months.<br />
<br />
Usually, I take the flight to Ottawa to see my kids sometime during July / August but this April thru May four events happen (in the near future):<br />
<ul>
<li>My daughter and her boyfriend bought a house in Fort McMurray (it is 5 hours by car north-north-east from Edmonton) and they invite me to see there new digs,</li>
<li>My wife is going to PEI (Prince Edward Island) for a conference about stroke... and I'll come with her as well, of course,</li>
<li>My son & his wife has the second baby, Evie, 4 months old. We have to visit in Ottawa to see the new baby!,</li>
<li>Minor details but I have minor surgery and will take approx. 2 days +/-.</li>
</ul>

Holy man, I think there is <b><i>more</i></b> 1) typing <a href="http://stroke-survivors.org/">SSTattler</a>, 2) surgery and 3) simultaneous travel, 1 car & 2 flights across the continent, at least for me. I decided that I will shorten the time to <b>Apr/09/2016 - Weekly Index from Jan/01/2011 to Apr/02/2016/ </b>and that is the end of <a href="http://stroke-survivors.org/">SSTattler</a>.<br />

Total == 5 years and 3 months. I will keep the site up until the end of December / 2016.</div>

I loved to generated the weekly <a href="http://stroke-survivors.org/">SSTattler</a> with great <a href="http://www.stroke-survivors.org/2010/01/articles-by-author-or-contributors.html">Guest Bloggers</a> and lots of articles <a href="http://www.stroke-survivors.org/search/label/%E2%8C%98%20Headline">Headlines with Wikipedia & Video</a> and, maybe, some person will do it for the future...<br />

<li>I started <a href="http://stroke-survivors.blogspot.com/">stroke-survivors.blogspot.com</a> in January 2011 with no title and very few ideas of how to generate something useful for stroke survivors. At least I could shorten the html to <a href="http://stroke-survivors.org/">stroke-survivors.org</a> (<a href="http://stroke-survivors.com/">stroke-survivors.com</a> cost lots of money!!). </li>
<li>My second problem after my stroke is <i>aphasia</i> - my english grammar is relatively awful. I can not generate the number of (correct) articles needed by day or by the week etc. I needed other authors but <i>where</i> are they...</li>
<li>Useful articles from Sharon (my wife), and a few free from the internet but there is a hole somewhere and I have to fix it but how... ?</li>
<li>General format is OK but it is a there is nothing <i>good</i> about it, to actually have readers to read my web site for stroke survivors. There a bunch of articles with really <i>no point at all</i>.</li>
<li>At the first year, at least, I could generate some <i>goals</i>:</li>

<li><a href="http://www.stroke-survivors.org/search/label/.Jackie">Jackie Poff</a> came in the <b>first</b> author, mostly she was the <a href="http://www.stroke-survivors.org/search/label/.Jackie">Jester</a> and later she found Canadian <a href="http://www.stroke-survivors.org/search/label/.RickMercer">Rick Mercer Report</a>.</li>

<li>Install <a href="https://analytics.google.com/" target="_blank">Analytics</a> from <a href="http://google.com/" target="_blank">Google</a> - great statistic and free if you needed only simple stats - I like free stats! <li>On January 7th 2012 , Sharon (my wife) told me about <i>comics for stroke</i>! I added 4 Comics <a href="http://www.gocomics.com/forbetterorforworse/2006/09/26" target="_blank">For Better and For Worse</a> (<a href="http://catalog.fborfw.com/indexstoryline.php?q=Stroke" target="_blank">Grandpa Jim had has a Stroke - 88 strips</a>), <a href="http://dilbert.com/strip/2011-12-27" target="_blank">Dilbert</a>, <a href="http://www.gocomics.com/garfield/2011/12/26" target="_blank">Garfield</a> and <a href="http://www.gocomics.com/pickles/2012/01/02" target="_blank">Pickles</a>. <ul>

The session I am involved in, looks at how digital technology is transforming healthcare. I will begin with the patient perspective and share my story as a stroke survivor and talk about the role of social media/digital in my life.<br />

At the end I will be challenging the panelists and audience to think about how such technology can be harnessed to better support patients. The challenge will be around how healthcare providers could offer stroke survivors a more personal and effective stroke recovery experience, freeing up resources that could be used elsewhere and what that future could look like.<br />
<br />
There will be an opportunity for questions at the end and there is also live Twitter feed so please support me and put this date in your diary along with the time and join me and everyone involved. I will let you all have the Twitter feed address as soon as I have it.<br />

Stress is your enemy.<br />

Stress is especially an enemy for caregivers - and those people are often prime targets for stress.<br />
<br />
The needs of caregivers can be woefully ignored, including their own health care, which includes mental care. You should take note from this story on how <a href="http://www.medpagetoday.com/Cardiology/Strokes/56652" target="_blank">stressed spouse caregivers race a higher stroke risk:</a><br />
<br />
▶ Spousal caregivers who reported being under moderate or severe strain had a 5.1% rate of incident stroke, compared with 2.6% among propensity-matched people who were not caregivers ... , Sindhu Lakkur, PhD, of the University of Alabama at Birmingham, and colleagues found. ...<br />
<br />
▶ The researchers suggested there could be a public health implication for targeting stroke prevention efforts to this higher risk group of struggling spouse caregivers. Exactly how this might be done needs further study, Lakkur told MedPage Today.<br />

Let's say you're a chronic stroke survivor. You know "chronic" – it's the time after your plateau. Typically the chronic phase goes from 3 months to the end of the stroke survivors life. Do you still have a chance to get better during the chronic phase?<br />

There's actually good scientific reason to believe that you can get better after the plateau – once you are chronic.<br />
<br />
And here it is…<br />
<br />
Typically survivors are forced through rehab as soon as possible. Facilities are expensive and home is cheap, so off you go! The problem is that to get the survivor out the door, <b>compensatory strategies</b> are implemented. Things like AFOs and using the good arm to get everything done. It makes sense, these things do get people out of the system. The problem is those same strategies also hurt the survivors brain.<br />

OK, but how does this all help the chronic survivor? It sounds like it would hurt recovery.<br />
<br />
Here's why: Since the survivor is encouraged to use the "good" side, the "bad" side of the brain sits fallow- doing nothing. The upshot is that many survivors have a huge portion of the brain, usually adjacent to the area damaged by the stroke, that is on vacation. You've heard of 'use it or lose it'? Well, they've lost it. BUT, it can be regained. How?<br />

Before I had a stroke I would have found my post "9 Things I Have to Do to Drive Independently" obsessively tedious. I know of only two groups of able-bodied people who can understand what you have to do to thrive after a stroke.<br />
<br />
<u>Astronauts.</u> Astronauts understand the conscious attention stroke survivors have to pay to familiar tasks that able-bodied people do without thinking. As I watched astronaut Scott Kelly demonstrate cooking and eating dinner on the International Space Station, I saw that he had a problem. He repeatedly dropped food because his hand forgot you have to attach an object to a surface or it will float away. To be fair, Scott was distracted by having to look towards the camera and explain what he was doing. I am sure he performs better when he does not divide his attention. I think he would understand why stroke survivors do not perform well when they try to multitask. I also think the months of practice Scott did on Earth would help him understand how hard it is for stroke survivors to relearn to do even simple tasks.<br />
<br />
<u>Urban Housewives in 1900.</u> If women in 1900 could have read the nine things I do before I drive my car away from the curb, they would have said "welcome to my life." They knew about spending lots of time getting ready to do a task. For instance, before they could start doing laundry they had to do three chores. 1) They had to soak clothes the day before because cleaning agents were not very effective. 2) They had to hand carry 20 to 40 gallons of water from the sink to a large copper vat where the clothes were washed. 3) They had to wait for the coal fire under the copper vat to heat up the water. Only then could they start agitating the clothes by hand with a four foot long pole called a dolly stick.<br />

Freedom by definition is: the power or right to think, speak or act as one wants. The state of not being imprisoned or enslaved. To sum up it's bloody fantastic!<br />
<br />
I love being able to get up, get ready and go out on my own. I walk along the pathway to my car, get in and then I'm off. Gone are the days when I would have to wait for my mum to finish work and come shopping with me. I am free to do what I want, WHEN I want.<br />
<br />
My little car Penny, (I'm probably the biggest Big Bang Theory fan so it's named after Penny in the programme and it's a girl car because it's all pretty and blue) plays a huge part, since learning to drive my independence has expanded. I love just getting up in the morning and driving to college like it's nothing major. It's just what I do.<br />
<br />
On friday I kinda got a little emotional driving on my way to physiotherapy. I just felt so in control saying bye to both my parents, getting in my car and driving all the way to Markyate (a journey quite long and one I never thought my parents would let me do on my own!!). I was 'driving' on cloud nine- I felt extremely lucky to have recovered well from a brainstem stroke, firstly. But also the fact that I stuck with learning to drive, passed first time and managed to get a car of my own. Yes, 'normal' people do it all the time but for a stroke survivor who was told she'd never walk or talk again, is pretty big!<br />
<br />
<a name='more'></a>Also, physically I'm more confident. Although when I'm shopping, I have a trolley to hold onto but so? I'm walking by myself. I walk all round the huge supermarket, I even go upstairs if I need to. I don't get fatigued as much now, my leg muscles are big enough to carry me round the shop without me having to stop to 'catch my breath'.<br />
<span class="Apple-tab-span" style="white-space: pre;"> </span><br />
And as we probably all know, trolleys have a mind of their own, there always seems to be one with a wobbly wheel... but I'm testing my balance and coordination- so technically it's part of this whole rehabilitation process.<br />
<br />
And for anyone that wants to go out on their own but is a little nervous or worried...<br />
<br />
- They <b>will</b> help you.<br />
If, like me, you have a weak arm and cannot pack your shopping, they will pack for you. The first time I went to a till and paid I imagined myself having to struggle with it all. But no. All the cashiers that I've been to will do it for you. And they're happy to help out.<br />
<br />
- Walk around like you <b>belong</b>.<br />
You have just as much right as anyone else to be shopping in that supermarket. People will look at you and they may step out of your way quicker than anyone else because they assume you can't push a trolley, but it's to be expected really. Just get on with your shopping, don't focus on anyone, do your thing.<br />
<br />
Independence, freedom. It's a huge thing. After nearly 4 years I can finally do things on my own again. So it does take time. But believe me, there is nothing greater than when you can finally take that little step on your own. Remember, what may seem small to others, is a HUGE accomplishment for you. Let no-one dull your shine.<br />

Environmental enrichment is the stimulation of the brain by its physical and social surroundings. Research on animals finds that environmental enrichment could aid the treatment and recovery of numerous brain-related dysfunctions, including Alzheimer's disease and those connected to aging, whereas a lack of stimulation might impair cognitive development. Moreover, this research also suggests that environmental enrichment leads to a greater level of cognitive reserve, the brain's resilience to the effects of conditions such as aging and dementia.<br />

Research also finds that attaining and engaging in higher levels of education, environments in which people participate in more challenging cognitively stimulating activities, results in greater cognitive reserve.<br />

<span style="font-size: large;">Early Research</span></h3>
<br />
Donald O. Hebb in 1947 found that rats raised as pets performed better on problem solving tests than rats raised in cages. His research, however, did not investigate the brain nor use standardized impoverished and enriched environments. Research doing this first was started in 1960 at the University of California, Berkeley by Mark Rosenzweig, who compared single rats in normal cages, and those placed in ones with toys, ladders, tunnels, running wheels in groups. This found that growing up in enriched environments affected enzyme cholinesterase activity. This work led in 1962 to the discovery that environmental enrichment increased cerebral cortex volume. In 1964, it was found that this was due to increased cerebral cortex thickness and greater synapse and glial numbers.<br />

Also starting around 1960, Harry Harlow studied the effects of maternal and social deprivation on rhesus monkey infants (a form of environmental stimulus deprivation). This established the importance of social stimulation for normal cognitive and emotional development.<br />

Rats raised with environmental enrichment have thicker cerebral cortices(3.3–7%) that contain 25% more synapses. This effect of environmental richness upon the brain occurs whether it is experienced immediately following birth, after weaning, or during maturity. When synapse numbers increase in adults, they can remain high in number even when the adults are returned to impoverished environment for 30 days suggesting that such increases in synapse numbers are not necessarily temporary. However, the increase in synapse numbers has been observed generally to reduce with maturation. Stimulation affects not only synapses upon pyramidal neurons (the main projecting neurons in the cerebral cortex) but also stellate ones (that are usually interneurons). It also can affect neurons outside the brain in the retina.<br />

<span style="font-size: large;">Dendrite Complexity</span></h3>

Environmental enrichment affects the complexity and length of the dendrite arbors (upon which synapses form). Higher-order dendrite branch complexity is increased in enriched environments, as can the length, in young animals, of distal branches.<br />

<span style="font-size: large;">Activity and Energy Consumption</span></h3>
<br />
Synapses in animals in enriched environments show evidence of increased synapse activation. Synapses tend to also be much larger. Gamma oscillations become larger in amplitude in the hippocampus. This increased energy consumption is reflected in glial and local capillary vasculation that provides synapses with extra energy.<br />

These energy related changes to the neuropil are responsible for increasing the volume of the cerebral cortex (the increase in synapse numbers contributes in itself hardly any extra volume).<br />

Part of the effect of environmental enrichment is providing opportunities to acquire motor skills. Research upon “<i>acrobatic</i>” skill learning in the rat shows that it leads to increased synapse numbers.<br />

Environmental enrichment during pregnancy has effects upon the fetus such as accelerating its retinal development.<br />

Environmental enrichment can also lead to the formation of neurons (at least in rats) and reverses the loss of neurons in the hippocampus and memory impairment following chronic stress. However, its relevance has been questioned for the behavioral effects of enriched environments.<br />

<span style="font-size: x-large;">Mechanisms</span></h3>
<br />
Enriched environments affect the expression of genes in the cerebral cortex and the hippocampus that determine neuronal structure. At the molecular level, this occurs through increased concentrations of the neurotrophins NGF, NT-3, and changes in BDNF. This alters the activation of cholinergic neurons, 5-HT, and beta-adrenolin. Another effect is to increase proteins such as synaptophysin and PSD-95 in synapses. Changes in Wnt signaling have also been found to mimic in adult mice the effects of environmental enrichment upon synapses in the hippocampus. Increase in neurons numbers could be linked to changes in VEGF.<br />

<span style="font-size: x-large;">Rehabilitation and Resilience</span></h3>

Research in animals suggests that environmental enrichment aids in recovery from an array of neurological disorders and cognitive impairments. There are two mains areas of focus: neurological rehabilitation and cognitive reserve, the brain's resistance to the effects of exposure to physical, natural, and social threats. Although most of these experiments used animal subjects, mainly rodents, researchers have pointed to the affected areas of animal brains to which human brains are most similar and used their findings as evidence to show that humans would have comparable reactions to enriched environments. The tests done on animals are thus meant to represent human simulations for the following list of conditions.<br />

<span style="font-size: large;">Autism</span></h3>
<br />
A study conducted in 2011 led to the conclusion that environmental enrichment vastly improves the cognitive ability of children with autism. The study found that autistic children who receive olfactory and tactile stimulation along with exercises that stimulated other paried sensory modalities clinically improved by 42 percent while autistic children not receiving this treatment clinically improved by just 7 percent. The same study also showed that there was significant clinical improvement in autistic children exposed to enriched sensorimotor environments, and a vast majority of parents reported that their child's quality of life was much better with the treatment. A second study confirmed its effectiveness. The second study also found after 6 months of sensory enrichment therapy, 21% of the children who initially had been given an autism classification, using the Autism Diagnostic Observation Schedule, improved to the point that, although they remained on the autism spectrum, they no longer met the criteria for classic autism. None of the standard care controls reached an equivalent level of improvement. The therapy using the methodologies is titled Sensory Enrichment Therapy.<br />

<h3>
<span style="font-size: large;">Alzheimer's Disease</span></h3>
<br />
Through environmental enrichment, researchers were able to enhance and partially repair memory deficits in mice between ages of 2 to 7 months with characteristics of Alzheimer's disease. Mice in enriched environments performed significantly better on object recognition tests and the Morris Water Maze than they had when they were in standard environments. It was thus concluded that environmental enrichment enhances visual and learning memory for those with Alzheimer's. Furthermore, it has been found that mouse models of Alzheimer's disease that were exposed to enriched environment before amyloid onset (at 3 months of age) and then returned to their home cage for over 7 months, showed preserved spatial memory and reduced amyloid deposition at 13 months old, when they are supposed to show dramatic memory deficits and amyloid plaque load. These findings reveal the preventive, and long-lasting effects of early life stimulating experience on Alzheimer-like pathology in mice and likely reflect the capacity of enriched environment to efficiently stimulate the cognitive reserve.<br />

<span style="font-size: large;">Huntington's Disease</span></h3>
<br />
Research has indicated that environmental enrichment can help relieve motor and psychiatric deficits caused by Huntington's disease. It also improves lost protein levels for those with the disease, and prevents striatal and hippocampal deficits in the BDNF, located in the hippocampus. Multiple studies have reported that environmental enrichment for adult mice helps relieve neuronal death, which is particularly beneficial to those with Parkinson's disease. A more recent study shows that environmental enrichment particularly affects the nigrostriatal pathway, which is important for managing dopamine and acetylcholine levels, critical for motor deficits. Moreover, it was found that environmental enrichment has beneficial effects for the social implications of Parkinson's disease.<br />

<span style="font-size: large;">Stroke</span></h3>

Research done in animals has shown that subjects recovering in an enriched environment 15 days after having a stroke had significantly improved neurobehavioral function. In addition these same subjects showed greater capability of learning and larger infarct post-intervention than those who were not in an enriched environment. It was thus concluded that environmental enrichment had a considerable beneficial effect on the learning and sensorimotor functions on animals post-stroke. A 2013 study also found that environmental enrichment socially benefits patients recovering from stroke. Researchers in that study concluded that stroke patients in enriched environments in assisted-care facilities are much more likely to be engaging with other patients during normal social hours instead of being alone or sleeping.<br />

A 2008 study found that environmental enrichment was significant in aiding recovery of motor coordination and some recovery of BDNF levels in female mice with conditions similar to those of Rett syndrome. Over the course of 30 weeks female mice in enriched environments showed superior ability in motor coordination to those in standard conditions. Although they were unable to have full motor capability, they were able to prevent a more severe motor deficit by living in an enriched environment. These results combined with increased levels of BDNF in the cerebellum led researchers to conclude that an enriched environment that stimulates areas of the motor cortex and areas of the cerebellum having to do with motor learning is beneficial in aiding mice with Rett syndrome.<br />

A recent study found that adult rats with amblyopia improved visual acuity two weeks after being placed into an enriched environment. The same study showed that another two weeks after ending environmental enrichment, the rats retained their visual acuity improvement. Conversely, rats in a standard environment showed no improvement in visual acuity. It was thus concluded that environmental enrichment reduces GABA inhibition and increases BDNF expression in the visual cortex. As a result, the growth and development of neurons and synapses in the visual cortex were much improved due to the enriched environment.<br />

Learning and spatial memory that are dependent on the long-term potentiation of the hippocampus are vastly improve as subjects in an enriched environments had lower levels of lead concentration in their hippocampi. The findings also showed that enriched environments result in some natural protection of lead-induced brain deficits.<br />

This has led to some suggestions that the spinal cord has a continued plasticity and all efforts must be made for enriched environments to stimulate this plasticity in order to aid recovery.<br />

Some children in such impoverished orphanages at two and half years of age still fail to produce intelligible words, though a year of foster care enabled such children to catch up in their language in most respects. Such children show marked differences in their brains, consistent with research upon experiment animals, compared to children from normally stimulating environments. Not only is higher education linked to a more cognitively demanding educational experience, but it also correlates with a person’s general engagement in cognitively demanding activities. The more education a person has received, the less the effects of aging, dementia, white matter hyperintensities, MRI-defined brain infarcts, Alzheimer's disease, and traumatic brain injury. Also, aging and dementia are less in those that engage in complex cognitive tasks. The cognitive decline of those with epilepsy could also be affected by the level of a person’s education.<br />
